Please complete the student dismissal form below based on how your child will be transported home to and from school each day for the 2025-2026 school year.
If you have more than one child attending HIES, please complete one form for each child.
Please send a note to your child’s teacher on any day that different dismissal arrangements are needed.
A hard copy of the student dismissal form will also be available to complete if preferred.
